Patients applying for special authority approval are asked to have their providers SA forms printed and with the patient information portion filled out to be signed by the provider should they be deemed a candidate. Due to confidentiality, it is the patients responsibility to send any communication to benefit providers.

The aesthetic follow up is ONLY for any aesthetic treatment that has been completed in the last 2 weeks. Please cancel with 48 hours notice if you don't need the appointment. A $50 fee will be applied for any no shows or late cancellations.
